01

What is Zabopegdutide (DD01)?

Plain language first. The technical label stays, but it never stands alone.

Overview

Fat building up in the liver, and the inflammation it can bring (MASH), can scar the liver over time. In a 67-person phase 2, 76% of people on weekly zabopegdutide cut their liver fat by at least 30% in 12 weeks, against 12% on placebo. At 48 weeks, MASH cleared on biopsy in 62.5% against 5.3%, but these are company topline figures from only 35 people and are not yet peer reviewed.

The technical version

Zabopegdutide is a 30-residue glucagon-based peptide with Aib at position 2, a glutamate 16 to lysine 20 lactam bridge and an added C-terminal cysteine amide carrying a large branched PEG; GSRS puts the conjugate at about 57 kDa against about 3.8 kDa for the peptide, so it is mostly PEG by mass. It activates GLP-1 and glucagon receptors, about 11 times more strongly at GLP-1 by the company's figures. The peptide matches efinopegdutide's except at positions 17 and 18, but it has no antibody Fc part; D&D Pharmatech develops it through its subsidiary Neuraly.

What it's used for

An experimental weekly injection for fatty liver disease (MASLD and its inflamed form, MASH) in adults with overweight or obesity. It is not approved anywhere.

02

How does Zabopegdutide (DD01) work?

What it does in the body, and where.

Zabopegdutide switches on two hormone receptors, mostly GLP-1 with some glucagon, from a peptide held in the body by a large PEG.

  1. 01
    The GLP-1 side
GLP-1 receptor activity lowers appetite, slows stomach emptying and improves insulin release when blood sugar is high. By the company's figures it is about 11 times more potent at GLP-1 receptors than at glucagon receptors.
  2. 02
    The glucagon side
Glucagon receptors on the liver raise fat burning and lower stored fat. The company links this to liver fat falling 37.4% in phase 2 patients who lost under 5% of their weight.
  3. 03
    The PEG
The large branched PEG slows absorption and clearance: levels peak about 6 days after a dose and the half-life is about 7 to 8 days, allowing weekly injection. Aib at position 2 resists the enzyme DPP-4, and a lactam ring holds the peptide's helix in shape.

03

What else is it used for?

Each group carries its own evidence level.

Approved means regulators have cleared it for that purpose. Investigational means it is in trials. Early research means it is being studied and is not established.

Liver fat at 12 weeks (phase 2)

Investigational
  • Human trial67 adults with overweight or obesity and MASLD or MASH: 76% (25 of 33) against 12% (4 of 34) cut liver fat by at least 30% on MRI (adjusted relative risk 6.3, P<0.0001).
  • Human trialSide effects in 85% against 68% on placebo, most often nausea, diarrhea and vomiting.

Deeper liver fat responses and weight

Investigational
  • Human trial72.7% against 5.9% cut liver fat by more than half, and 48.5% against 0% reached normal liver fat (under 5%); the placebo figures come from the company's June 2025 release.
  • Human trialWeight fell 3.8% by week 12 and 6.4% by week 24 on the drug; in those who lost under 5% of their weight, liver fat still fell 37.4%.

Liver biopsy at 48 weeks

Investigational
  • Human trialMASH resolved without worsening fibrosis in 62.5% against 5.3% (p=0.0003); fibrosis improved by at least one stage in 50.0% against 15.8% (p=0.0323); both in 37.5% against 5.3%.
  • No dataCompany topline from a per-protocol group of 35 (16 of 33 on the drug, 19 of 34 on placebo), about half of those enrolled, not yet peer reviewed.

Phase 1

Investigational
  • human-early4 weeks of weekly doses in adults with obesity, type 2 diabetes and fatty liver: the company reported a mean liver fat fall of more than 50% at the two highest doses against under 5% on placebo, with falls even at doses that caused no significant weight loss.
Often left out

It is often called simply an investigational peptide, but it is a 30-residue peptide joined to a large branched PEG: GSRS puts the whole molecule at about 57 kDa against about 3.8 kDa for the peptide, and the company has not stated the PEG's size. It is also reported as too small and short to show longer-term results, but the trial ran 48 weeks and the company released biopsy results in May 2026; those come from a per-protocol group of 35 and are not peer reviewed. A '37.0%' fall in liver fat at 12 weeks has been reported for the whole group, but the company's 37.4% applies only to people who lost under 5% of their weight; the group average was 62.3% against 8.3%. Stopping for side effects is reported as 4 in total with 3 on the drug, while the abstract summary gives 4 on the drug against 1 on placebo. The phase 1 is given as 107 people by the company and 255 in its registry. The human half-life is 7 to 8 days; 11 days came from monkeys. Early materials called it 'imbalanced' and later ones 'balanced', but the company's 11:1 GLP-1 to glucagon ratio makes it GLP-1-weighted. It is not efinopegdutide: the peptides differ at positions 17 and 18, and efinopegdutide's PEG ends on an antibody Fc part.

How do you store it?

Before mixing, and after.

Dry powder in the freezer or fridge. Once mixed, fridge, and use within about a month.

Unmixed lyophilized Zabopegdutide (DD01) keeps for a long time cold. Once you add water the clock starts: most reconstituted vials hold up for roughly 28 to 30 days at fridge temperature. Keep it out of light, and do not freeze it after mixing.
