What is Substance P?

Plain language first. The technical label stays, but it never stands alone.

Overview

Substance P is a pain and inflammation signal, and the useful way to understand it is to look at what approved medicine does with it. The answer is block it. The anti-nausea drugs given during chemotherapy are NK1 receptor antagonists, meaning they stop substance P from acting. In October 2025 the FDA approved elinzanetant for hot flashes in menopause, and its mechanism is described by its maker as inhibition of substance P and neurokinin B. Meanwhile a research group in Denmark is currently recruiting people with migraine to receive substance P by infusion, for the explicit purpose of triggering migraine attacks. Against all of that sits a single 2019 laboratory paper proposing substance P in a gel as an anti-aging skin ingredient. Its three authors all work for the same company, no human skin was tested, and the paper says so in its own discussion.

The technical version

Substance P is an 11 amino acid tachykinin, residues 58 to 68 of the 129 amino acid protachykinin-1 precursor, reading RPKPQQFFGLM and amidated on the C-terminal methionine at position 68. The same precursor also yields neurokinin A at residues 98 to 107, neuropeptide K and neuropeptide gamma. Substance P is the principal endogenous agonist at the neurokinin-1 receptor, where it mediates pain transmission, neurogenic inflammation, vasodilation and the vomiting reflex. Its pharmacology in approved medicine runs entirely in the blocking direction: every licensed drug acting on this system is an NK1 receptor antagonist. It has no approval as a therapy in any country.

What it's used for

Nothing. It has no approval anywhere. It is given to humans only in research, and in the current studies the purpose is to provoke headache or migraine in order to study the mechanism.

02

How does Substance P work?

What it does in the body, and where.

This page is built around a contradiction in directions. Approved medicine blocks substance P; one paper proposes applying it. Reading the evidence on each side settles which is which.

  1. 01
    One precursor, four peptides
The protachykinin-1 precursor is 129 amino acids long and is cut into several active peptides. Substance P is residues 58 to 68, an 11 amino acid chain reading RPKPQQFFGLM with its terminal methionine amidated. Neurokinin A comes from residues 98 to 107 of the same precursor. Neuropeptide K and neuropeptide gamma are longer products of the same gene. A source describing effects of tachykinins in general is not necessarily describing substance P.
  2. 02
What it does, and the direction medicine took
Substance P is the main natural agonist at the neurokinin-1 receptor. Through it, substance P transmits pain, drives neurogenic inflammation, dilates blood vessels and participates in the vomiting reflex. Every drug licensed on this system blocks that receptor rather than activating it. The aprepitant family prevents chemotherapy-induced nausea and vomiting by blocking NK1. In October 2025 the FDA approved elinzanetant for menopausal hot flashes, and its mechanism is stated by its maker as inhibition of substance P.
  3. 03
What happens when it is given to people now
The Danish Headache Center is currently recruiting for a trial titled Substance P-Induced Migraine Attacks Without Aura. Participants with migraine receive a continuous intravenous infusion of substance P at 4.0 picomoles per kilogram per minute over 20 minutes, and the primary outcome is the incidence of migraine attacks in the 12 hours after the infusion starts against placebo. The purpose is to trigger attacks. That is the current state of substance P administration in humans: a provocation agent.
  4. 04
    The anti-aging paper, read properly
A 2019 paper in the International Journal of Molecular Sciences is titled Anti-Aging Potential of Substance P-Based Hydrogel for Human Skin Longevity. Three things about it matter. All three authors are at one company's research institute. The work is entirely laboratory work: cultured human fibroblasts, isolated keratinocytes and melanocytes, and a reconstructed three-dimensional skin model. No human subject was tested. The authors state this themselves in the discussion: the anti-aging effects of SP gel have not yet been demonstrated on human skin, and they describe a human irritation test as future work. A title containing the words Human Skin Longevity, over a study that tested no human skin, is the sort of gap worth noticing.
  5. 05
    Holding the two sides together
There is no contradiction in the underlying biology. Substance P promotes inflammation, and inflammation drives both the pain the approved drugs treat and the collagen response the laboratory paper measured. The question is which effect dominates on a living person's face, and no study has answered it. Given that the approved uses of this system all involve blocking substance P, the burden of proof on applying it sits high.

What else is it used for?

Each group carries its own evidence level.

Approved means regulators have cleared it for that purpose. Investigational means it is in trials. Early research means it is being studied and is not established.

Approved medicine blocks it

FDA-approved
  • RegulatoryElinzanetant was approved by the FDA on October 24, 2025 for moderate to severe vasomotor symptoms due to menopause. Its maker states the mechanism as inhibition of substance P and neurokinin B through NK1 and NK3 antagonism.
  • RegulatoryThe aprepitant family of NK1 receptor antagonists is licensed to prevent chemotherapy-induced nausea and vomiting, again by blocking the receptor substance P acts on.
  • No dataNo approved drug supplies substance P. Every licensed agent on this system is an antagonist.

What it is currently given to people for

Early research
  • human-earlyAn actively recruiting trial infuses substance P intravenously at 4.0 picomoles per kilogram per minute in people with migraine, with the incidence of migraine attacks as the primary outcome. The aim is to provoke attacks.
  • No dataNo trial gives substance P as a treatment for anything.

The anti-aging claim and what supports it

Research only
  • Lab studyOne 2019 paper proposes a substance P hydrogel as an anti-aging skin ingredient. All three authors work at the same company's research institute.
  • Lab studyThe study used cultured cells and a reconstructed skin model. No human subject was tested.
  • No dataThe authors write in their own discussion that the anti-aging effects have not yet been demonstrated on human skin, and that a human irritation test remains future work.
  • No dataWe searched for finished cosmetic products containing substance P and did not find one offered for sale.
Often left out

The anti-aging framing is the error worth catching. It traces to a single 2019 laboratory paper whose authors all work for one company and who state in their own discussion that the effects have not been demonstrated on human skin. The paper's title contains the phrase Human Skin Longevity, and the study tested no human skin, so a citation of the title alone badly overstates what was done. The second error is directional. Readers encountering substance P as a wellness ingredient are usually unaware that the only approved drugs touching this system block it, including a menopause drug approved in October 2025 whose stated mechanism is inhibition of substance P. Third, substance P is sometimes conflated with tachykinins generally or with neurokinin A; those come from the same precursor gene but are different peptides with different sequences. Last, a 10-residue sequence beginning with proline circulates in some listings, and the reference range is 11 residues beginning with arginine.

Searched: substance P migraine, substance P provocation, substance P hydrogel, substance P skin aging, elinzanetant.

Left out: substance P (the bare name returns on the order of tens of thousands of records across pain, inflammation, gastroenterology and neuroscience, swamping the two narrow questions this card addresses); tachykinin (the family term, covering neurokinin A and neuropeptide K from the same gene); neurokinin A (a different peptide from the same precursor, at residues 98 to 107); aprepitant (a marketed antiemetic with its own substantial literature); NK1 receptor (a receptor term dominated by antiemetic drug development); SP (an abbreviation matching an enormous amount of unrelated text).

Minutes, not precisely establishedNo figure

No reliable human half-life figure was found in the sources read for this page. Native substance P is degraded very rapidly, and that is the practical reason research suppliers stock modified analogs more often than the natural sequence. No number is quoted here rather than giving one we could not source.

Nothing to store: it is not taken as a treatment.

Substance P is not used as a medicine or a supplement, so no home storage question arises. Laboratory suppliers ship research material frozen and state their own handling conditions on the certificate that comes with it.

Who should avoid it

Check these before anything else.
  • Any skin or anti-aging use: the one supporting paper is laboratory work by a single company and states in its own discussion that the effects have not been demonstrated on human skin
  • Any use at all, since the direction of every approved drug acting on this receptor is to block substance P rather than supply it

09

Is Substance P approved?

And where the numbers on this page come from.

No. Substance P is not approved for any use, anywhere. It is sold labeled for laboratory research only, and there is no legal route to buy it for human use.

What has happened recently

Not approved anywhere as a therapy, and the regulatory activity around it runs in the opposite direction. The drugs licensed against this system are all NK1 receptor antagonists: the aprepitant family used to prevent chemotherapy-induced nausea and vomiting, and now elinzanetant, approved by the FDA on October 24, 2025 for moderate to severe vasomotor symptoms due to menopause. Bayer describes elinzanetant as the first dual neurokinin targeted therapy and states its mechanism as inhibition of substance P and neurokinin B through antagonism of NK1 and NK3 receptor signaling. That is the clearest possible statement of where the therapeutic value lies. Substance P itself is sold as a research reagent. We searched for finished cosmetic products containing substance P and did not find one, so the anti-aging proposal below appears to be a published idea rather than something currently on sale.

Guides for this compound

Sources

  • UniProt P20366, human protachykinin-1(2026)
The reference sequence record, read positionally. The precursor is 129 amino acids. Substance P is annotated as residues 58 to 68, and counting the sequence from position 58 gives RPKPQQFFGLM, 11 residues, with a modification at position 68 recording the methionine amide. Neurokinin A is residues 98 to 107, giving HKTDSFVGLM with its own amide at 107. Neuropeptide K is 72 to 107 and neuropeptide gamma is annotated in two non-contiguous parts. Worth noting for anyone checking a vendor datasheet: a 10-residue version of substance P beginning at proline is a transcription error, since the annotated range begins one residue earlier at arginine.
  • Trial registry record NCT06959004, substance P migraine provocation trial(2026)
Titled Substance P-Induced Migraine Attacks Without Aura: A Randomized Clinical Trial, sponsored by the Danish Headache Center with the principal investigator at Rigshospitalet. Recruiting, started May 13, 2025, estimated enrollment 21. The intervention is a continuous intravenous infusion of 20 mL of substance P at 4.0 picomoles per kilogram per minute over 20 minutes, against isotonic saline on the same schedule. Primary outcome is the difference in incidence of migraine attacks without aura between substance P and placebo during the 12 hours after the infusion begins, with headache intensity as a secondary outcome. The stated purpose is to examine whether substance P can trigger migraine attacks.
  • Kim, Chang and Lee, International Journal of Molecular Sciences, the anti-aging hydrogel paper(2019)
Anti-Aging Potential of Substance P-Based Hydrogel for Human Skin Longevity, volume 20, issue 18, article 4453, published September 10, 2019. All three authors are listed under a single affiliation, the Research and Development Institute at Biosolution in Seoul. The work is in vitro: human dermal fibroblasts, keratinocytes and melanocytes isolated from tissue, and a reconstructed three-dimensional skin model. No in vivo human testing is reported. The discussion states verbatim that the anti-aging effects of SP gel have not yet been demonstrated on human skin, that additional studies would be required to clarify the anti-aging effects, and that the authors will perform a primary irritation test using SP gel on human skin in the future. Reported findings are more collagen than substance P alone, an anti-inflammatory effect, improved absorption, and no induction of melanin synthesis in co-culture. DOI resolved at Crossref.
  • Bayer press release, FDA approval of Lynkuet (elinzanetant)(2025)
Dated October 24, 2025, announcing FDA approval that day for the treatment of moderate to severe vasomotor symptoms due to menopause, taken orally once daily. Describes the drug as the first dual neurokinin targeted therapy, an NK1 and NK3 receptor antagonist, and states the mechanism as inhibition of substance P and neurokinin B through antagonism of NK1 and NK3 receptor signaling on KNDy neurons. Cited here because it is the clearest regulatory statement that the therapeutic direction on this system is to block substance P.
  • Firazyr (icatibant) prescribing information, cited for the contrast(2026)
Included to make the same point from a second angle. Icatibant is described in its own label as a selective, competitive inhibitor of the bradykinin B2 receptor, used for acute attacks of hereditary angioedema at 30 mg subcutaneously. Bradykinin and substance P are different peptides on different receptors, and in both cases the approved medicine is an antagonist rather than the peptide itself.
