No published human study establishes a dose for this blend. The range comes from vendor and community practice, and is anchored to one component because a blend is one draw: the other components follow at whatever ratio is in your vial.
Tesamorelin's approved daily dose is between 1.28 and 2 mg depending on which of the three products you compare against. Reaching 2 mg from a 10 mg and 3 mg vial also delivers 600 mcg of ipamorelin, about twice its commonly cited 200 to 300 mcg. From a 5 mg and 5 mg vial it delivers 2 mg of ipamorelin, closer to seven times that. The ratio you happen to buy changes the ipamorelin dose by more than three-fold.

How do you store it?
Before mixing, and after.Dry powder in the freezer or fridge. Once mixed, fridge, and use within about a month.
Unmixed lyophilized Tesamorelin + Ipamorelin blend keeps for a long time cold. Once you add water the clock starts: most reconstituted vials hold up for roughly 28 to 30 days at fridge temperature. Keep it out of light, and do not freeze it after mixing.

What has happened recently
Tesamorelin on its own is FDA-approved as EGRIFTA, EGRIFTA SV and EGRIFTA WR, for HIV-associated lipodystrophy. Ipamorelin is not approved for anything, and was moved to FDA compounding Category 2. The blend has never been tested in a human trial. An approval that one ingredient holds does not carry over to a combination it was never studied in.
